Muncie, IN – Open Door Health Services was recognized, once again, with a Level 3 Certification as a Patient Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA). A Patient Centered Medical Home is an innovative care model that replaces the reactive, episodic care often practiced in the current medical system with a proactive, coordinated care model designed to improve the patients’ health and moderate long-term health care costs. Open Door is the only Level 3 PCMH in East Central Indiana. This honor recognizes the proactive and coordinated care delivered by the 170 employees of the organization.
To qualify for PCMH status, Open Door was required to show proof that they provide and maintain a high level of patient engagement and outreach, effectively removing barriers to care. NCQA conducted a rigorous review of Open Door’s focus on improving patient outcomes by evaluating everything from the availability of same-day appointments to support for patients in the self-management of their chronic diseases.
